Verification Forms

For the following Word document links, you could type on the Word document, save it and email it to registrar@nnu.edu in an attachment, or you could print the form and send it to the address printed on the directions.

 

Request for Verification (Fillable-Word)

 

To receive an official verification of enrollment letter please fill out and sign the form (click on Request for Verification above). 

 

Request for Degree Completion Letter (Fillable-Word)

 

To receive a verification of degree completion (between completion of requirements and the posting of the degree) please fill out and sign the form (click on Request for Degree Completion above). 

 

Request for Non-Attendance Letter (Fillable-Word)

 

To receive a verification of non-attendance (you never enrolled/attended or are no longer enrolled) please fill out and sign the form (click on Request for Non-Attendance Letter above). 

Change Forms

Please click on the 'Sample' version link that is above the DocuSign link to preview the form.  If you are ready to initiate the form, click on the DocuSign link.  You must click on 'FINISH' to send the form to the next person on the recipient list.  Please don't go into the DocuSign form if you aren't going to complete it.

The graduation application is also available in paper form in the Registrar's office - Emerson Administration Building, Room 10B (downstairs).
