This form may be completed by any member of the Northwest Nazarene University community who has experienced or witnessed discrimination, discriminatory harassment, or retaliation as defined in NNU’s Nondiscrimination Policy. All information provided will be forwarded to the Title IX coordinator.

The questions on this form are optional. The information provided will be used to track reports of discrimination, discriminatory harassment, and retaliation for statistical monitoring and reporting purposes to federal authorities. The information will also be used by the University to better understand the incidence of discrimination, discriminatory harassment, and retaliation in our community, and to aid the University in its education, prevention, and response efforts.
